Most recent Form 990 or financial statement
Organizational Mission Statement
A brief summary of the program or project in need of funding
Budget breakdown for the requested funds
Contact information for your organization's primary grant liaison
Any relevant photos, testimonials, or success stories (optional, but encouraged)

Grants are awarded twice a year - June (deadline April 30th) and December (deadline October 30th). Applications received after the deadline will not be considered.
